III. Contraindications

  1. Hypersensitivity to Proton Pump Inhibitors
  2. Use with caution in severe liver disease
    1. Limit to maximum dose of 15 mg/day

IV. Dosing: Adults

  2. Background
    1. Generic, OTC
    2. Take 30 minutes prior to a meal
    3. May open capsule and sprinkle on applesauce, cottage cheese, yogurt or in juices (apple, orange, tomato)
      1. May be given via NG tube
  3. Gastroesophageal Reflux
    1. Take 15 mg orally daily for up to 8 weeks
  4. Erosive Esophagitis
    1. Take 30 mg orally daily for 4 weeks, then 15 mg orally daily for up to 6 months
  5. NSAID-Induced Gastric Ulcer
    1. Take 30 mg orally daily for 8 weeks, then 15 mg orally daily for up to 3 months
  6. Zollinger-Ellison Syndrome
    1. Take 60 mg orally daily

V. Dosing: Children

  1. Delivery
    1. May be compounded into liquid for dosing in infants
    2. May sprinkle opened capsule onto food or into juice
    3. Available in a disintegrating tablet
  2. Weight <10 kg (and age 3-12 months)
    1. Dose: 7.5 mg twice daily or 15 mg daily
    2. Dose: 1 mg/kg/day (0.5 to 1.6 mg/kg)
  3. Weight 10 to 30 kg (age 1 to 11 years)
    1. Dose: 15 mg orally daily for up to 12 weeks
  4. Weight >30 kg (age 12 to 17 years)
    1. Dose: 30 mg orally daily for up to 12 weeks

VI. Pharmacokinetics

  1. All PPIs have short plasma half life of 1-2 hours
  2. Effect is delayed 5-7 days until proton pumps are fully blocked
  3. Metabolized by CYP2C19 and CYP3A4
  4. Inhibits CYP3A4
