- Suspected Substance Abuse in Pregnancy
- Mechanism
- Meconium accumulates drug exposure throughout pregnancy
- In utero exposure of drugs implied by positive test
- Exposure may have been months prior to delivery
- General
- Lab testing methadology is critical to efficacy
- Screening Tests are non-specific
- Confirmatory testing is highly specific
- Gas chromatography mass spectrometry (GC/MS)
- Liquid Chromatography mass spectrometry (LC/MS)
- Efficacy
- False positives if infant urinates into sample
- Reflects antepartum and perinatal esposure
- False negatives
- Meconium sample at room temperature
- Decreases Cocaine and cannabinoid by 25% per day
- Preterm Infant
- Drug Screening (sensitive, but not specific)
- Opiates
- May be positive due to codeine, heroin, morphine
- Less sensitive for Oxycodone and hydrocodone
- Heroin clears very quickly
- However 6-acetylmorphine metabolite clears slowly
- Cocaine
- Multiple metabolites may be measured
- False positive is very rare
- Theororetically Lidocaine can cause positive
- In reality, Lidocaine ammount required too high
- Amphetamines
- Methamphetamine metabolized to Amphetamine
- MDMA (Ecstacy) metabolized to MDA
- MDA and Amphetamine may appear similar on testing
- Cannabinoids (Marijuana)
