  1. Suspected Mononucleosis with negative heterophile tests
  2. Mononucleosis-Like Syndrome

III. Physiology

  1. IgM Viral Capsid Antigen (xIgM VCA or EBV IgM)
    1. Present at onset of symptoms
    2. Persist for 4-8 weeks after onset
  2. IgG Viral Capsid Antigen (xIgG VCA or EBV IgG)
    1. Present early in symptom course
    2. Persist life long
  3. Epstein-Barr Virus Nuclear Antibody (xEBNA)
    1. Suggests past infection
    2. Not present until 6-8 weeks after symptom onset

IV. Interpretation

  1. Acute phase antibodies: xEA, xIgM VCA
  2. Convalescent Antibody: xIgG VCA
  3. Recovered State: xIgG VCA, xEBNA
