II. Definitions

  1. Organ failure
    1. Shock or
    2. Hypoxemia (paO2 60 mmHg or less)
    3. Serum Creatinine >2 mg/dl
    4. Gastrointestinal Bleeding >500 ml/24 hours
  2. Local complications
    1. Pancreatic necrosis
    2. Pancreatic Abscess
    3. Pseudocyst
  3. Early prognostic signs (systemic complications)
    1. Ranson score 3 or greater
    2. APACHE II Score 8 or greater

III. Severity: Mild Acute Pancreatitis

  1. No organ failure
  2. No local complications (e.g. peri-pancreatic fluid collection, pancreatic necrosis)
  3. No systemic complications

IV. Severity: Moderate Acute Pancreatitis

  1. Transient organ failure (<48 hours) or
  2. Local complications or
  3. Exacerbation of comorbidity

V. Severity: Severe Acute Pancreatitis

  1. Persistent organ failure (>48 hours)

VI. Interpretation

  1. Mild Acute Pancreatitis
    1. Mortality <1%
    2. Resolves within 1 week in most cases
  2. Severe Acute Pancreatitis
    1. Mortality approaches 30%
