  1. Standard Adult Dosing
    1. CrCl >=50 ml/min: 4 g IV every 8 hours administered over 3 hours
  2. Renal Dosing
    1. CrCl 30 to 49 ml/min: 2 g IV every 8 hours administered over 3 hours
    2. CrCl 15 to 29 ml/min: 2 g IV every 12 hours administered over 3 hours
    3. CrCl <15 ml/min: 1 g IV every 12 hours administered over 3 hours
      1. Administer after Hemodialysis
