II. Indications

  1. Constipation Dominant Irritable Bowel Syndrome

III. Contraindications

  1. Mechanical Bowel Obstruction
  2. Children (esp. age < 6 years)
    1. Increased risk of severe Dehydration

IV. Mechanism

  1. Locally acting Sodium-Hydrogen exchanger 3 Inhibitor (NHE3 Inhibitor)
    1. Increases intestinal fluid secretion

V. Dosing

  1. Very expensive ($1500/month in 2022)
  2. Take 50 mg orally twice daily immediately before breakfast and supper

VI. Efficacy

  1. May reduce Abdominal Pain and improve stool frequency (NNT 9 to 12)
    1. Similar efficacy as Lubiprostone and Linaclotide

VII. Adverse Effects

  1. Abdominal Distention
  2. Flatulence
  3. Dizziness
  4. Diarrhea (16% of cases, severe in 2.5% of cases)
    1. Hold for severe Diarrhea
