- Intravenous: 15 mg/kg/day IV divided tid

- Oral: 8-20 mg/kg/day divided tid to qid
- Dosing: Adult
- Oral: 150-450 mg PO qid
- Intramuscular: up to 600 mg PO q8 hours
- Intravenous: 600 to 900 mg IV q8 hours
