Autosomal Dominant Polycystic Kidney DiseaseAka: Polycystic Kidney Disease, ADPKD
- Epidemiology
- Prevalence: 500,000 patients in United States
- Symptom onset most often after age 30 years
- Pathophysiology
- Type 1 (86% of cases, 95% of cases in Caucasians)
- ADPKD1 gene on short arm of chromosome 16
- End-stage renal disease average age of onset: 69
- Children of affected patients have 50% risk
- Type 2 (14% of cases)
- ADPKD2 gene on chromosome 4
- End-stage renal disease average age of onset: 57
- Symptoms
- Flank or Abdominal Pain (60%)
- Signs
- Hypertension
- Labs
- Urinalysis
- Hematuria
- Radiology
- Renal Ultrasound
- Abdominal CT
- Diagnosis
- Adults
- Three or more renal cysts
- Bilateral renal parenchymal involvement
- Child
- Two renal cysts
- Involvement may be unilateral or bilateral
- Complications
- Renal Failure (45%)
- Accounts for 5-10% of End Stage Renal Disease (ESRD)
- Hypertension
- Children: 30%
- Adults: 60%
- End Stage Renal Disease: 80%
- Hematuria (associated with renal cyst rupture)
- Recurrent Urinary Tract Infection
- Nephrolithiasis (20%)
- Renal Cell Carcinoma
- Cerebral Aneurysm (5-10%)
- Adult polycystic liver disease - liver cysts (34-78%)
- Mitral Valve Prolapse (26%)
- Colonic diverticulum with risk of perforation
- Risk Factors for End Stage Renal Disease
- ADPKD1 gene
- Early onset at young age
- Male
- Black race
- Sickle Cell Trait
- Hypertension
- Left Ventricular Hypertrophy
- Gross Hematuria
- Proteinuria
- Large renal volume

- Three or more pregnancies
- Management
- Hypertension control (Goal Blood Pressure < 130/85)
- ACE Inhibitor
- Diuretic (caution when used with ACE Inhibitor)
- Calcium Channel Blocker
- Hematuria: Most resolve spontaneously
- Maximize hydration
- Correct coagulopathy
- Bed rest
- Proteinuria (30%)
- Flank and Abdominal Pain with cyst expansion
- Analgesics
- Rest
