  5. Dosing: Child
    1. Preterm Infant
      1. Intravenous: 15 mg/kg/day IV divided tid
    2. Term Infant and Child
      1. Intravenous: 20-40 mg/kg/day IV divided q6-8 hours
      2. Oral: 8-20 mg/kg/day divided tid to qid
  6. Dosing: Adult
    1. Oral: 150-450 mg PO qid
    2. Intramuscular: up to 600 mg PO q8 hours
    3. Intravenous: 600 to 900 mg IV q8 hours
