  1. Dosing
    1. Amenorrhea
      1. Sequential: 5 to 10 mg qd for 5-10 days
      2. Continuous: 2.5 mg qd
    2. Dysfunctional Uterine Bleeding (DUB)
      1. Sequential: 5-10 mg qd for 5-10 days, start on Day 16
    3. Postmenopausal Estrogen Replacement (with Premarin)
      1. Sequential: 2.5 to 5 mg PO qd for last 10 cycle days
      2. Continuous: 5 to 10 mg qd
  2. Adverse effects
    1. Major Depression
    2. Breakthrough bleeding
      1. Increasing dose to 20-30 mg qd may reduce bleeding
        1. However may have increased risk
        2. Long term lipid and cardiovascular effects?
